SPECIALISING IN NURSING & RESIDENTIAL CARE IN NOTTINGHAM

We have two admission processes into Connect House. Our admissions onto the Garden Suite is via City Care and the triaging hub. Our residents in this suite are those who have typically experienced a hospital stay and who need a short-term placement prior to going home or on to long-term care. Connect House do not take admissions directly for this service and all admissions go via a triaging process. At the point of discharge planning in hospital, each individual will be asked if they wish to go to Connect House for further reablement and assessment. These services are carried out by City Care staff.

Admission onto our Heritage suite is via ourselves. We are duel registered and this means we can accept both those family members with residential care needs and those with nursing care needs. Prior to an admission, we carry out an assessment process using a nursing model based upon the “Activities of Daily Living’ (Roper, Logan and Tierney, 1982) to help us to access a person’s care needs and determine if we can meet these needs.

During this process we focus on a person’s identity and life history. Where possible, a team member will spend face to face time with the person to help us to understand their individual needs and to help us to keep them central to all decisions being made around their care and treatment.

When we accept a new family member into the home, we are doing so having considered not only their needs as an individual and whether we can meet these needs, but also having considered the needs of those family members already living at Connect House.

Connect House is set in the residential area of Basford close to shops, parks and a little further a field, Nottingham City Centre. We have good transport links and are a short 5 minute walk from Nottingham City Hospital.
